How to add check boxes into a matrix question and a text/graphic?

Is it possible to add a check box next to a statement on a matrix question? I have tried to do this using html code <input type="checkbox"> and it is showing in the edit survey view (see image below) but it does not show when I publish and view the survey properly. I also need to add some checkboxes at the start of a section of questions, and again have tried doing this using the html code on a text/graphic and it is showing in the edit survey view (see below) but not when I view the actual survey.  I am a complete noob and have never used the javascript or embedded data before but have seen both of these things mentioned in responses to other similar questions so I am not sure if I need to use these. Is anyone able to help with this? Edit: I have now changed the layout to Simple and the checkboxes are now visible. Do I need to do anything else to ensure that the checkboxes record data?

Yes since you are in Simple Layout use Javascript to store the checkbox check status in an embedded data.
