That is essentially a Bipolar Matrix with four scale points.

Exact same would not be possible but as TomG mentioned A bipolar Matrix which would look like below. I hope this works for you. It will be auto single select only from the 4 scale points.

Yes, I’ve successfully delivered my survey via Qualtrics. I’ve used @Deepak’s and @TomG’s recommandations. I have not receive any questions regarding this format (being really close to the original questionnaire).

Did you use the survey for a research study to be published? If so, is there a paper out there to show reliability of the online version.
