I am seeking guidance on how to best replicate the following type of table, where column 1 is piped text from a previous question, and the respondent must answer mutliple questions about each selection. (And the number of piped items in column 1 can range from 1 - 4 responses).  See photo:

 

I would suggest using the Side-by-Side question type. This allows you to have multiple columns of questions, each with their own set of answers. Looking at the example you provided, I think that is the behavior you’d like to have replicated. 

 

Below is a quick preview of what it would look like. You can have the choices as either radio buttons or drop-down selection (High School, Current). There may be a way with JSON to have the radio buttons display vertical instead of horizontal to try and condense the spacing. 

You would then need to set display logic on your statements (column 1 in your image) to show only if that option is selected in the previous question.
