I'm creating a survey for my thesis and I need respondents to answer with ONE answer to a multiple choice question (like a Likert scale). But the last choice is "choose if the statement worries you" and I need to make that choice available too. I thought about the multiple choice variant but that wouldn't do the trick I'm afraid.
(I'm assessing if the Ages and Stages' Social-Emotional (Second Edition) 12-months questionnaire is suitable for estimating the social-emotional development of Estonian toddlers.)

You can create a multi-select question just below your likert question. Do not add page break so both can be shown on same page. Now add a custom validation just like SS below so that multiselect answer act as exclusive. You can add javascript also to handle validation here.
