Is it possible to design a survey with 1 question where the choices of the question re-directs participants to a different Qualtrics survey to complete? and the re-directed survey will record their responses on the individual project on Qualtrics and not the initial screening survey?

You should be able to use Branching in the Survey Flow to do this. Link an End of Survey object to each choice from your question, and customize the options, redirecting to the URL of each survey.
