So the Insurance company I am trying to get registered with is saying this to me to complete: 

Currently, we are unable to process your application due to
missing Assignment Account Paperwork for EMIA Wellness LLC, Tax
ID#85069****, NPI#141757***, Location#138 N Mountain Blvd, MountainTop,
PA 18707. Please visit Online Survey Software | Qualtrics Survey
Solutions and complete the Request for Assignment Account form. Once
completed please respond to this email with the confirmation number so I
can have the request expedited.

Where do I find the Request for Assignment Account Form???

Did I get it right that you are no Qualtrics user but want to register with you business for some issurance and they sent you a mail which contains:

„Please visit Online Survey Software | Qualtrics Survey“?

If I got it right…

Please understand that Qualtrics has nothing direct to do with any insurance company but is some software to build surveys. It seems like the insurance you applied for is using Qualtrics to build their „request for assignment“.

Doesn‘t the text mentioning the survey have a link? If not, you should reach out directly to the insurance company and ask your question. Qualtrics will not be able to assist here…
