Hi 

I have a question in my survey which is how many children do you have - Depending on the response I have set up blocks/branches which all works fine.

What I need is that participants are able to answer each question with a ‘subheading’ for each child

Ideally to look like the following:

Do your children like carrots?

Child 1:

yes 

no

unsure

Child 2:

yes

no 

unsure

In some of the questions, there needs to open text boxes again for child1, child2, child3 etc (depending on the amount of children selected.

Very grateful for any help, or a nudge in the right direction on options to look at.
