Answers were automatically choose in one block

image.pngHi, I have a problem when creating survey. I created several multiple choice (single answer) questions in one block.
But when I previewed the survey, when I choose one answer, it will automatically apply to all questions in that block.
I've tried to delete the whole block and re-enter the questions. But it didn't work. How do I fix this?

      I've never ran across this. Sounds like a bug or some sneaky JS.
