Audio file does not appear

If you used code, please paste the code you used. * Are you seeing this behavior only in preview or also in the full survey mode? * Does the audio appear but not play or does it not appear at all? * Do you get any error messages?

I have inserted the file as a question with the following code-<audio class="qmedia" controls="true" height="40" preload="auto" width="320"><source src="https://qtrial2019q2az1.qualtrics.com/CP/File.php?F=F_1zZ0UbaRnYMFzEh" type="audio/mp3" /><embed class="qmedia" flashvars="file=https://qtrial2019q2az1.qualtrics.com/CP/File.php?F=F_1zZ0UbaRnYMFzEh&width=320&height=20&type=mp3&autostart=false" height="20" pluginspage="http://adobe.com/flashplayer/" src="/WRQualtricsShared/Flash/flvplayer.swf" type="application/x-shockwave-flash" width="320" wmode="transparent" /></audio> This does not appear both in the preview and the final survey mode (i have tried to publish the survey and it did not appear there as well) it does not appear at all and there are no error comments.
