Can I create a question with 2 columns and 2 tick boxes either side?

Hi,
I am trying to input a questionnaire from the Resistance to Peer Influence questionnaire (screenshot below) but cannot find this format on Qualtrics.
This is the question style. I have tried using Matrix table and then Max Dif but this only allows one column in the middle and one tick box either side. Does anyone know how I could format questions like this on Qualtrics?

    You can use Matrix Table Likert, Multiple Answer question type. Instead of text in middle it'll show on left side and then using simple JavaScript code you can get that text in middle.

    Thanks for your answer. How do I add the java script? Is this possible on the free version?

    Unfortunately free version doesn't allow adding JavaScript code.
