Create a Task in Actions to Send a Survey once a JIRA Ticket Status is Set to Done

Since I was not able to find this information here, or any where else for that matter, for the next person who needs it, I thought I would post the steps here (in a PDF). This is for connecting JIRA with Qualtrics, using Webhooks, in the Survey Project > Actions > Editor tab.
