Is there any way to validate the survey responses so that only one response gets selected from the entire row? Or would there be a better way to format this?

I wouldn't use a side-by-side...just use a Likert Matrix with labels:

Thank you! Can you walk me through how added labels to your likert. When I do it the formatting is inconsistent with the numbers being out of line.

You can't rely on how it looks in the editor. Preview it and it will look like the example I posted.
