Have a dropdown list called "Name" with names of the people taking the field survey. When I run a report the names appear as N/A, I need the name with the date for a two month assessment using the survey form.

Yes name is a multiple choice, i want the person to taking the survey to select their name so the company can track how many surveys an individual has inputted. The problem is the name on the report shows N/A. I have Other as a name choice with logic to enter the first and last name if they are not listed. This name shows up on the report, thanks for your help
