I want to have an option at the start of my survey where I ask the end-user to enter their email id so that a personalised link is generated for them and they can click on the link to start the survey.

for example- Enter your email address -
Once the user enters his email address, I want a message to be displayed saying that thank you for registration. A personalised link should be generated there and then the user can take the survey
