How do I create a report from the embedded data I used to create unique URLs for a recurring survey?

From another post, I got the idea to create embedded data with no values to create unique URLs so I can have a recurring survey, but have all the data together to have easier access to parse it in different ways. I created the embedded data points (with no values, like that articles said), two unique URLs, and gathered a few sample collection responses. I am now stuck at the point of whether that actually gave me anything useful. My embedded data are two dates (I thought numbers would be easy to do and continue) and I want to be able to identify which responses were for which 'date.' It didn't seem to work. Any help in making this possible would be much appreciated.
