How do I open up specific Likert scale items based on participants answers

Hi,
I'm having trouble designing the following option:
My participants will fill a list of items in a likert scale format.
I'm looking for an option that only the items that were marked in the scale point 4 and 5 for example would open up in the next question.
There participants will be asked to fill some information in a text box in form format for each statement they marked 4/5.
Any tips? I've tried playing with the piped text but I can't figure the way to do it in a simple and efficient manner.
It is important that all the questions they marked 4/5 would appear together instead of a different question opening up for each one in order to save time.
