How to create a matrix table with radio button field and text field
I need to create a table similar to the attached picture. The first four columns will be radio buttons but the last column needs to be a text field. I am using a matrix to create this table but I do not see a way to add a text field as the last column. Is there anyway to do this.

Take a look at side-by-side response. You can add open ended text as a column option.

Agreed with @WaterSampler. But in best practice, it is said that each column is one question in SBS question type. Hence you need to be very careful while mapping data in data analysis tab, as the comment column will have separate data column
