I can't send my distribution

The "Send Now" button is grayed out when I try to distribute a survey, regardless of what I put for the "To" or "From" fields. This has never been an issue in the past. Why is this happening? Screenshot attached: (http://take.ms/ctvdL)").

I believe it may be because you have an error to resolve. . . . "No opt out link"

Ah yes, I do see that - this is one of my first distributions...any idea how I add an opt out link to get rid of that error?
