Respondents can either upload an image or fill in a text entry
I would like to ask respondents to either upload an image or describe the image using text entry. How can I have both answer options for the same question or do I need to split up the two types of answers?

The option to show both together under one question is currently not available within Qualtrics.
We had a similar need for one of our surveys. We ended up showing two questions on the same screen/page - one question was open-ended asking for more information and the second question was asking to upload an image. You can create a forced response for both if you are needing respondents to answer both before moving on.

Another way would be to first ask them to upload an image (if that's the most important one) but also give them the option "I don't have an image". If they select the non-image option or do NOT upload an image (easy logical checks), on the next page you can use display logic to display the text entry question.
