I want to do a custom validation on this question. If they say yes, I want to force them to put in a number larger than 0. If they say No, they have to leave the number box blank. Is this doable?
I guess this would not work with side-by-side design. It should work with multiple choice though. You could put a text entry box under each item and display it whenever an items is answered "yes". And then force to put in a number larger than 0.

For instance, if you weren't alive during the Presidency of Jimmy Carter, and Clicked No, then the Other Choices to Rate or add Comments should be disabled.
