I got several responses from last survey showing as "anonymous", and I am curious whether these are spams.
I am attaching image on the bottom.
How do I prevent getting these types of issue in the future?
Is there any security setting that I could check?

In Survey Options select "By Invitation Only."
Why is this happening?
What does "By Invitation Only" do? Would it block any regular process (email survey)?

If you allow anonymous, anyone with the survey link can take the survey. If you set your survey to "By Invitation Only" they have to use a personal link (email invite or generated personal link) to take the survey.
