Evaluation survey for individual contributors (w/ multiple hierarchical/functional appraisers)

Hi there, What is the best tool available to run a performance appraisal of several individual contributors of an organization, being necessary to collect at least two evaluations (one from hierarchical manager + one from functional manager). I am wondering if there is any option that enables to set-up a predefined list of surveys (one by individual contributor) to be answered by each different appraiser.
