How do I add different scale points for different options in the same question?

Hi all,
Very new Qualtrix user here.
In a matrix type question, I have four options to be rated on a scale of 1-5. The first there options have same scale points (1= least to 5=most) but the last one has different points (1= exclude and 5= include). (See the attached image)

I am not sure if this can be done. If yes, could you please please suggest how.

In the Matrix question, change matrix type to profile.
