Is there any way to have a report on sub-tasks when it comes to Action Plan Reporting?

I am looking for a way to report on action plans that includes the action plan creator ID, action plan creator, the action plan title, any adjoining sub-tasks, and associated any comments. I have found a way to report on all of these, except sub-tasks. Does anyone have some reporting tips they can share on how to pull this data?
