how best to get families with multiple children to commit to in-person or virtual modality

Just wondering if anyone has good ideas or templates to gather information from families about what type of modality they want for their students (in-person or virtual). I know I can have the parents submit a commitment for each student individually, but was wondering if there was a clean way to have them be able to enter multiple students in one submission. We did this before with Google Forms and the spreadsheet on the back end was awful.

Any help would be appreciated.

We did something similar with a loop and merge. Use the attached QSF as a template, you should be able to change the wording/type of a couple questions and have most of what you need.
