📂 QSF Template: Athletics Forms Collection and Registration template

This template was developed to automate the athletics registration workflow at our 3 HS. It automates the collection of the necessary forms and the required parent and student signatures. The survey depends on our student contact list that is loaded automatically nightly from our Infinite Campus SIS with key information such as Guardian names phone numbers and addresses. The survey prepopulates that information so the guardian just confirms the information is correct when registering therefore dramatically reducing the data input required. This along with a CE dashboard allows the athletic staff to track and distribute key roster information to coaches as registrations are completed.
