Parents use this survey to indicate their child's absence. Through the use of survey flow and a ticketing system, our district nursing staff are notified of anyone displaying COVID-like symptoms while the attendance secretaries at each school are able to see all student absences. We utilized a JavaScript to give parents the option to mark their child absent for multiple days using a calendar interface. Survey actions are used to send an email to each parent's email address we have on file alerting them that their child has been marked absent.
