📂 QSF Template: Counseling-Student Suicide Ideation Inventory (w/XM Directory)|Omaha Public Schools

Attached is a new survey we are piloting right now for counselors conducting a suicide ideation inventory. Basically, they have a lot of protocols depending on how a student answers the items so we standardized the process in Qualtrics so that all counselors are given the same guidance. Additionally, we use the directory to import a student list each week and a student number authenticator to record student information and then we will be pulling that information periodically and writing it into our information system for records (something our IMS team helps me execute). Then a copy of the inventory will be emailed to the counselor for their records and a basic overview of the incident (suicide ideation survey) will be recorded for the student for us to review later. You will definitely want to edit/remove our contact information from this but I think it’s a great project which could be replicated at different schools based on their policies.
