📂 QSF Template: COVID-19 Student Testing Consent

We created the COVID-19 Student Testing Consent when we received the rapid testing option for students on campus. The school nurses were trained to provide the testing, however we needed to have parent consent before the test can be administered. This form allows the parent to sign the consent form. Once the form is completed it is automatically emailed to the specific campus nurse as well as our Health Services department.
