We frequently use this form for collecting parent signatures for field trips - we make a copy of this template, insert the appropriate details at the top of the page, and then can send out the link via Class Dojo/ParentSquare/etc. and also print a 8.5”x11” flyer with a QR code link to the survey that we can put up on the front door and in the office for parents to easily access the survey.

I’d also recommend setting up a quick report for the school principal/office manager/etc. that gives a listing of which students have replied - bonus points if you set up this report to give the parent contact info and the medical info field so that the school can simply print off the report to have everything they’d need on hand should it prove useful.
