This particular survey utilizes the Table of Contents feature and Display Logic to be able to have the a mentor fill out information about mentoring activities that they are doing with the teachers they mentor. Only the questions that pertain to the mentors that they have will show. Be aware that much of the survey depends on embedded data that comes from the contact list for each mentor. When the survey is completed, a report of all activities is emailed to the principal and the mentor.
