Does anyone have any surveys they have used to poll their community around their school? Not just parents and staff but the actual community in which the school is located?

+51We used one for our community regarding changing the names of some our school district buildings. One of the questions asked if they were a community member, staff member, student (or multi-select). I created the survey, but our communications manager implemented it. I believe they provided the link and QR code on our district webpage(s), our SIS information blast, social media, our school board information, and some community groups to pass the option to participate as widespread as possible.
