Hello, I’m creating a simple survey with 3 questions but I was informed that folks can’t put their name to the survey.  They want to know who filled the survey out.  Is this possible?   Thank you. 

Hi @lhaynes456, do you have an audience list to whom you will send the survey? If so, you can import the list with their bio/demo information and then export it on the back end after the survey closes.
