I have a question about some advanced logic I have not worked with before. You can see my question below! 

 

Q1. Have you, or has any member of your family seen either a nurse practitioner or a physician in the last 6-12 months?

  1. Nurse Practitioner Only → SKIP TO Q2b, then SKIP TO Q3b
  2. Physician Only → PROCEED TO Q2a, them SKIP TO Q3a, then SKIP TO Q4
  3. Both Nurse Practitioner and Physician → PROCEED TO Q2a
  4. None of the above → SKIP TO Q4
  5. Not Sure → SKIP TO Q4

 

How, if possible, would this be done? Help on this would be great! Thanks!

It's single select question, use diaplay logic instead of skip.

  • Display logic at Q2a and Q3a. Q1=b or Q1=c
  • Display logic at Q2b and Q3b. Q1 = a or Q1=c

Your all conditions will be covered. Please test it properly for each scenario.

Do each of those questions need to be in individual blocks or can they all be included in the same block of questions? Thanks!

Add all questions in same block an use page break between them.

Hi @BradleyHoush ,

They can be any where but after the question on which Display logics is based(page break compulsory).
