I need to upload information about my survey in order for participants to participate in my survey. Can I upload my informed consent form and have them electronically sign it to move forward in my survey?

Ofcourse you can upload information/consent form to the survey, otherwise all surveys would be blank. What do you mean?
If you are looking for something like an authenticator/e-signing, then you'll need to use external JS libraries.
I am in the survey area adding questions just fine, but I want to upload the informed consent from my university to the survey at the beginning. Can I upload the exact template to the survey?
