How to disable other options if a particular option is selected {multiple choice- multiple answer question}

I’m creating a survey in which some questions are multiple choice, multiple answer. But for some of the questions, I don’t want the participant to be able to select the other options if 1 particular option is selected. 
Here’s a brief example:
 

-What type of opportunities are you looking for? [check box for multiple choice answers]
 
Self-employed

Employee

Contractor 

I’m not sure

I prefer to not disclose

If the participant selects, I’m not sure, I don’t want them to be able to select the other options. What is the best way to  do this?
