Set up a Qualtrics survey as an assessment/ test?

I'm looking to potentially set something up where I can embed a document into my Qualtrics solution (like a downloadable attachment). I then want to set my Qualtrics survey up as an assessment/ test on the content in that document. In the assessment/ test, the user would answer a set of questions and at the end, find out their score (if they then answer- say 80%- correct, they receive a 'congratulations' message. If the user doesn't meet the 80% and fails the assessment, they're given an opportunity to take it again.)

Is this possible? If so, how might one set this up?
