Confidentiality Statement for Survey Participants
Does anyone have a boilerplate statement with regard to keeping what participants see private or not to be shared? This would seem to be important especially for Product Concept Testing.

I often use this: "This survey and any files transmitted within it are confidential and intended solely for the use of the individual or entity to whom they are addressed. If you have received this survey in error please notify the system manager. This survey contains confidential information and is intended only for the intended individual. If you are not the intended participant you should not disseminate, distribute or copy this survey or its contents."
