How to create a question within a matrix containing both numerical and categorical options per item

Hello all,
I'm putting out a survey and one of the questionnaires has slightly complicated question items. Each item requires two responses: the first is a 0-10 rating (e.g how much or intense something was), and the second is categorical (i.e ticking a box). I'm not sure how to create multiple responses like this within a matrix in Qualtrics. Any ideas?

    Hi DominicResearch ,
    Have you looked into using a Side-by-Side question type? It's not mobile friendly but it would seem to meet your requirements.
