We have recently launched a simple COVID-19 self-assessment tool at the university. Respondents are required to enter their full name and email address with each survey entry so that we can both log responses and for the respondents to receive results.
There is some concern that members of our community are not completing the survey due to the inconvenience of having to enter their personal information every time. Is there a simple way to connect this information to a particular respondent so that those fields autopopulate every time they complete the survey? I could see how creating individual collectors or survey links might work but am open to any and all options.

How are respondents accessing the survey? Is there a link posted somewhere or is it sent by email? If the latter you can pipe the data from the email list. If the former, it would require some type of API connection with a database.
