Is it possible to design a survey like this?
I am trying to recreate the below survey in Qualtrics. I was wondering if anyone had any suggestions or tips on how to do this.

You could use a Likert Matrix with:
* Statements: Before this training, After this training, etc. (repeat as many times as needed)
* Assign statements to groups: ability to actualize..., ability to manage..., etc.
* Answers: 1 - 5
* Labels: Low, Medium, High
I'm not sure if this is possible. But you can break out the questions and run it as a loop and merge.
