You can check taking preview link in the different browser and different device... if its an issue for the different browser then please report it to Qualtrics support team.
+43Check by analyse and publish survey. Refresh browser. It happens sometimes not always. Try taking more survey run and confirm if it is still happening.

Update: contacted Qualtrics user support, and we learned that something about JavaScript in *adjacent* questions was messing up display in this question. Had to use different JavaScript in adjacent question to make this question display properly.
