Check by analyse and publish survey. Refresh browser. It happens sometimes not always. Try taking more survey run and confirm if it is still happening.
You can check taking preview link in the different browser and different device... if its an issue for the different browser then please report it to Qualtrics support team.
Update: contacted Qualtrics user support, and we learned that something about JavaScript in *adjacent* questions was messing up display in this question. Had to use different JavaScript in adjacent question to make this question display properly.
