"Not applicable" AND "Refuse to answer" options in Slider question

I'm trying to create a slider question type but besides the slider I also need two answer options: "Not applicable" AND "Refuse to answer". I am only able to get one through the "not applicable" option, but I cannot get a second box. Any ideas how to get it done?

Hi FloNeb - we'd recommend creating an additional question below your slider as your "refuse to answer" option. You can then setup Custom Validation to make sure the survey taker answers the slider question OR answers the "refuse to answer" option below.
