Please can someone tell me how to have radio buttons and text as options in a survey response as shown in the image below. What I'm saying is while there has been radio button option I want an additional option whereby the can add text for additional comments within a radio button.

Use a side-by-side question where the first column is your Yes/No radio button and the second column is text.
@Apashea In addition to @TomG 's suggestion, you can also add a separate question which would be displayed only if the respondent selects that radio button. It's typically a better option from an accessibility perspective, since matrix questions tend to be harder for assistive devices to pick up.
