Trouble Formatting Questions

I am new to Qualtrics, and I can't figure out how to format a Likert-scale question with the question flush left and above the choices/scale. The only option I've seen is to have the statements be on the left with the ratings at the top or right beside the statement. I want it to look like this except with the bubbles to choose underneath the numbers:
Volunteer as a tutor or mentor with youth from an underserved and underprivileged group.

mtanner I would use a Multiple Choice Question where you change the position from vertical to Horizontal & make sure label position is Above and not side (you can make these choices in the menu on the right):
