Why are the questions in my block appearing on different pages?
+1I have a block with 12 multiple choice questions. I haven't inserted any page breaks, but when i preview my survey each of the 12 questions are coming up individually as opposed to all at once. I used "carry forward choices" so that I wouldn't have to repeat responses for each item in the measure. Could this be a reason why I can't get the items on one page?

Yes, your understanding is correct. Page break is created when there is carry forward
