How to use repeated appearance of labels in slider question?

Hi community,
For my survey research I have a slider question included multiple statements. I would like to add the labels of the slider on top (likely Likert; see photo attached) before each statement. For example, for the slider of the statements "ambient humidity" and "caffeine consumption".

There's an option called "Add Labels" on left hand side. You can add no of labels and then define them based on your requirement via css or JS. Hope this helps!
