Good morning! So if I want to collect data from employees on a survey, such as work location city & state. Does this mean I need to fill out the GPO/1PA form? I need the survey to be completely anonymous.

Dealing with survey anonymity setups can definitely get a bit confusing, especially when internal compliance forms get brought into the mix.

Based on how Qualtrics handles this natively, there isn't any standard documentation that requires you to fill out a "GPO/1PA" form just to run an anonymous project. Instead, the actual mechanism for keeping everything completely anonymous relies entirely on your distribution setup and survey flow. Typically, you'll want to use the standard anonymous link found right in your survey options security settings. If you still need to capture demographic-style data or organizational hierarchies without linking it back to specific employees, you can configure your survey flow to nest those specific demographic question blocks so respondents can self-report that information while keeping their identity totally private.

If your organization has specific compliance requirements around a "GPO/1PA" form, or if you're hitting a wall trying to configure your survey flow blocks to keep things locked down, it might be a good idea to have a specialist take a look at your exact setup.
