add a custom date field

Hi, I am wondering is there a way to add a custom text entry field for capturing the date in this format mm/yyyy: 

I would like the text entry field to have the following, 2 digits for the month and 4 digits for the year:  

_ _ / _ _ _ _ 

Is there custom code to program the text entry field like so?
