+38yaozhao, rondev has given you a valid answer to this question. You should follow his recommendation. It matches the format you requested in your picture above. If you feel weird about the question just saying "Comments:" then expand upon it some - although just using "Comments" is common practice in the surveys that I design for UW. It doesn't offend our respondents and it actually saves them time of having to read overly verbose text - it is a way of showing respect to your respondent although it might seem curt. I support rondev's recommendation and if you need something more substantial than comments recommend you label the open text question:
"Please provide any comments you have about scheduling programs. (optional)"

Thank you! I use this method and it looks well. I also "add page break" before and after these 2 questions (or 1 question according to the original survey). Do you recommend this option? I think by adding page break before and after can force these 2 questions are shown up on the same page.
+38yaozhao yes, I agree you should use page breaks the way you described. It gives the respondent extra context if they decide to provide a comment (e.g. they can see their answers and what the comment is supposed to be about to write a meaningful answer in the open end). I'm a big fan of using page breaks over the questions per page function. This allows you to control the breaks so that a page has flow and purpose to it. Being thoughtful in your page breaks enhances the respondent experience.
